Anxiety becomes a problem when it stops being an occasional warning signal and starts shaping your decisions, your sleep and your sense of what you can manage. It takes many forms. Persistent worry, panic attacks, social anxiety, health anxiety, and specific phobias are all expressions of anxiety, each with their own pattern but sharing a common thread in the way they are maintained and the way they respond to treatment.

A useful way to understand anxiety is that it tends to involve two things happening at once: overestimating the threat in a given situation and underestimating your capacity to manage it. The result is that even ordinary situations can feel genuinely dangerous. Therapy addresses both processes, helping you to look more accurately at what you are actually facing and to build confidence in your ability to cope with it.

OCD, for example, involves intrusive unwanted thoughts that cause significant distress, followed by repetitive behaviours or mental rituals aimed at neutralising that distress. Panic disorder involves recurrent panic attacks with persistent worry about when the next one will strike. Social anxiety centres on intense fear of scrutiny or humiliation in social situations. Each of these responds well to CBT, though the specific approach is shaped around the difficulty in question.

CBT is the most well-established treatment for anxiety, and it works by looking closely at the cycle that keeps the difficulty going: the thoughts, the physical sensations, and the things we understandably do to feel safer that often keep the problem alive. Therapy helps you understand that cycle in your own case and then test out different ways of responding, at a pace you set.

The aim is not to remove all anxiety, which would not be realistic or useful, but to bring it back to a level where it no longer runs the show.
